PRODUCT WARRANTY

PHOENIX HABILITATION TECHNOLOGIES warranties that all products of its manufacture as follows: Devices will be free from defects in materials and workmanship for a period of 90 days from the date of fabrication. Warranty will be activated should the device(s) become defective or fail during the 90 days All defective components of the device produced or manufactured by PHT will, at PHT option, be either repaired or replaced at no charge to the customer should they become defective or fail during the 90 days.  No other warranty is expressed or implied and our liabilities are limited to the cost of the defective material or product only.  Our warranty tolerances are +/- ¼” in height and length, material thickness +/-.03” from customer supplied specifications.

Device under warranty claim must be returned to PHT for review prior to any credit being authorized.

Devices will be made to the written specifications provided by the customer and design flaws or changes by the customer are not considered a defect or failure.  Therefore, no warranty will apply to devices changed from the original work order instruction.

 

TURN AROUND TIMES

Our in-house production time will start on the next business day for all devices arriving after 10:30 a.m.: not including weekends and holidays.  Our standard in-house fabrication time is 5-7 days for our standard products, non-standard products and high-volume times of year do change fabrication thru put. If you have a tight/firm need by date, please contact us prior to shipping to ensure fab times. Should you need a device completed as a RUSH, please call the PHOENIX HABILITATION TECHNOLOGIES office at 847-752-5728 prior to shipping. There will be an Expedited or Rush Fee for services that need a faster turnaround time.  Less than 3 days (not including arrival day) is classified as “Expedited Turnaround” - there is a $50 upcharge per device and $75 for a pair. Same day fabrication (made the day it is received, ships the next day) is classified as Rush service - there is a $100 upcharge per device and $150 upcharge for a pair.

 

NEED TO KNOW'S

-FORMS (Web forms always up to date)- We require the use of PHT forms. Incomplete forms can incur a consultation charge $$ to convert or finalize the order form to maintain the warranty.  Practitioners are encouraged to call with questions to learn how to properly fill out forms at no cost. 

-CAST HEIGHT – due to the type of manufacture we utilize, we require that the cast be 1” higher than desired finish height.  Lesser heights will incur charges and warranty will be VOID if this extension exceeds 2”. Extension of casts will be an up charge.  

Fabrication Tolerances are + - ¼” or .635 cm

-CAST QUALITY-  PHT reserves the right to refuse to manufacture a device from a sub-par cast.

PHT only accepts fiberglass casts for manufacture. Failure to remove interface material results in an upcharge per cast. If cast is destroyed upon removal of interface a recast WILL be required.

PHT reserves the right to refuse to fabricate any devices not in accordance with its procedures.

 

SPECIAL COMPONENTS  OR NON-STOCK TRANSFERS

 All non-stock components can incur up to a 30% up-charge and up to 5 days or more delay in manufacture times.  A call is required before sending a cast if a transfer is not shown on our website. There are times additional transfers will be available that are not listed on the website and so a call my be advantageous.

THERE ARE SOME COMPONENTS WE DO NOT DO, SO PLEASE CALL! e.g. -  NYU TYPE FOOTPLATE, GAFFNEY JOINTS, ECT, RULE OF THUMB: IF IT DOESN'T HAVE A MOLDING DUMMY, WE MOST LIKELY DON'T DO IT.    SO PLEASE CALL TO ASK!!
